Coordinating Severe Chronic Illnesses for Palliation and Quality of Life

Nearly half of healthcare resources currently go into management of people with progressively severe chronic conditions. Various innovations work to improve the situation of these individuals, including hospice care, palliative care consultation, social services support, supportive housing, patient-centered care plans, and caregiver support
This session will review national best practices. The presenter, Dr. Joanne Lynn, was previously the Bureau Chief, Chronic Disease and Cancer, at the District of columbia Department of Health. Prior to that she was the Director for the Washington Home Center for Palliative Care Studies and a Researcher with RAND Health. She is a national expert in Palliative Care and End-of-Life concerns.

Locally, the Whatcom Palliative Care Partnership and the Critical Junctures Institute are working collaboratively to establish a multidisciplinary team of specialists who will work with other healthcare providers to relieve suffering and improve the quality of life for patients with advanced illness.
